1 Voluntary informed consent

2 Permitted checks
- Identity and right-to-work verification; employment and reference checks; qualification or licence checks; address or fraud checks where proportionate.
- Criminal record screening with informed consent and a role-related assessment. A record is not an automatic exclusion unless law or genuine safeguarding requirements require it.
- For child-facing services, legally required or appropriate checks involving the National Register for Sex Offenders and National Child Protection Register, completed through authorised processes.
3 Fair process
Before an adverse decision based materially on a check, I will be given an appropriate summary, an opportunity to identify error or context, and information about review, except where law restricts disclosure. Decisions must be relevant, proportionate, non-discriminatory and recorded.
4 Retention and sharing
Results are restricted to authorised reviewers and retained only for the published period. The platform should display a status badge and date, not sensitive report details. Information may be shared with authorities only where lawful or necessary to protect a person.
5 Declaration and signature
I confirm that information submitted is true and that I have read the Privacy Notice. Consent may be withdrawn for future checks, but withdrawal does not require deletion where lawful retention or an active safety investigation applies.
